*If you are a student looking for answers regarding your student account, please do not submit this form and instead contact the Office of Financial Aid.


The form will allow the requestor to upload attachments, if the requestor is providing a list of students to the Office of Financial Aid to obtain data about, OFA will need each student to have submitted a Student Consent Form. Requestors can copy and paste this link into their application. Completed forms will automatically be routed to OFA under the student's CSU ID. There are other circumstances that OFA might request a release form from each student.

Requests can take 1-2 weeks depending on other priorities within our office but we will try our best to get the data back to you by your desired date.

The Office of Financial Aid adheres to all Federal, State, and Institutional policies on the sharing of FAFSA data and FTI data. In most cases, Federal rules related to the sharing of FAFSA data and FTI data supersede any other policies on the sharing and use of this information.

Due to the Family Education Rights and Privacy Act, Higher Education Act of 1965, and IRS Tax Code Laws data requests could be denied or altered to comply with these laws and to protect the student's rights to Privacy. This could mean that the Office of Financial Aid would require each student to provide explicit consent for your office to receive the data you are requesting.

These laws and institutional policies could change from year to year and while we may have been able to provide data in the past does not mean we are able to provide the same data in current or future requests.
